The Consumer Needs Assessment questionnaire (on-line) can be found at this link. Please encourage your clients to complete this survey on-line. If they have limited access to the Internet, you may wish to get the questionnaire completed in person. Please go to FoundatioNet here for further information and here for the PDF version of the questionnaire. The response rate to this survey will depend heavily on our outreach to consumers, so let’s all do our part and get as many questionnaires completed as possible! All responses must be received by Friday, April 5! In person questionnaires must be scanned and emailed to NA@efa.org.
